SWPCA was started in 2013, after the loss of our founder Frances Davison's mother, who was put on Hospice. After this experience she found a passion for end of life care. Frances aims to serve her community by helping those who cannot get to another provider, or it is difficult. Until recently there was no practice focused on home-bound patients. Helping others by serving has always been her goal. Based upon her desire to have continuity of care for her home-bound, chronically ill patients, and not lose them when they needed to be referred to Hospice, Frances chose to add Hospice services in 2017, and has since added a Primary Care clinic on 3rd Street and our newest location in the foothills also provides Primary Care.
